TriHealth is pleased to announce we are now offering you access to your TriHealth Urgent Care medical records through TriHealth MyChart. The TriHealth MyChart patient portal is easy to navigate and gives you access to:

  • Communicate with your doctor: Get answers to your medical questions from the comfort of your own home.
  • Access your test results: No more waiting for a phone call or letter – view your results and your doctor’s comments within days.
  • Request prescription refills: Send a refill request for any of your refillable medications.
  • Manage your appointments: Schedule your next appointment, or view details of your past and upcoming appointments.
  • Set Health Reminders: You can set reminders for appointments and medications, helping you stay organized and proactive about your health.

How to Create a TriHealth MyChart Account

An online account with a MyChart username and password is required in order to use the TriHealth MyChart features. Please just follow the instructions below to create a TriHealth MyChart account.

Sign Up Guidelines On Desktop/Computer

Step 1: If you clicked on the sign-up link in an email from “Donotreplymychart@trihealth.org,” you will be taken to https://mychart.trihealth.com/mychart/Authentication/Login where you will be prompted for an activation code.

Should you have not received a link via email:

  • Visit https://mychart.trihealth.com/mychart/Authentication/Login to access the TriHealth MyChart login page.
  • When it displays the login page on the screen then
  • Click the blue “Sign Up Now” button now.

Step 2: Kindly enter the 15-digit activation number you received under “Step 1.” You will receive an email confirming the setup of your account after it is completed.

Please be aware that TriHealth’s records must match your Date of Birth and Social Security Number (SSN). If your SSN is missing or not provided, it can be entered as “0000” or “0001.” Please register for an account without an activation code if you are unable to log in.

To register online without an activation code, click on ”Sign Up Online” link. Complete the given information, which a third party identity vendor will verify.

Step 3: To verify your identification, respond to the third-party identity vendor’s collection of questions (example provided here; questions will pertain to your personal background). Once identity has been verified, click the link to receive a code sent to your email for last-minute confirmation. To proceed, enter the code.

Step 4: Setting up Two-Step verification: You may choose to continue or turn off this additional account security measure.

Step 5: Then choose and set up your MyChart Username and Password.

Step 6: Lastly, select and verify your choices for notifications.

Sign Up Guidelines On Mobile Devices

Step 1: Using your device (smartphone, tablet, etc.), Follow the same prompts as outlined above to open or download the MyChart app.

Step 2: The above outlined steps also applies to mobile devices, but you must download the TriHealth MyChart App first. Then create an account either with an active Activation Code or without one.
